Signing and keeping a copy
I voluntarily agree to sign electronically by typing my private signing name and selecting the intent-to-sign checkbox for each document. I can read every document before signing, print or save a copy, and access signed copies in my portal. Perspect records the document text and version, my account and role, signature, choices, server time, and a SHA-256 record digest. The digest helps detect changes; it is not an independent identity certificate. Contact the administrator through the portal to request a paper process or withdraw electronic consent for future documents.
Remote session privacy
Sessions may use online video, phone, or chat as chosen. Use a private setting; confirm the coach’s alias and appointment. Connectivity or service failures can occur. Perspect does not provide round-the-clock support. Members and coaches may not record, screenshot, transcribe, stream, or publish a session, message, or identifiable participant without separate explicit permission and applicable legal review. No recording permission is granted by signing this form.
Fees, cancellation, and conduct
Members pay $49 once for a welcome package and booklet; individual sessions are $45 for 30 minutes. Monthly plans are $210 for 8, $420 for 16, or $630 for 24 half-hour credits, renewing monthly until canceled. Package credits expire at the end of the paid period and do not roll over. Individual/welcome credits have no scheduled expiry. Cancel a booking at least 24 hours in advance to restore its credit within its original expiry; contact the administrator for late cancellations or service problems. Canceling renewals does not automatically refund earlier payments. Respectful, honest interaction is required. No result or particular coach/time is guaranteed.
